Your First Visit

Thank you for choosing Highline or Southwest Hand Therapy.

Please call to schedule an appointment. Please leave contact information on our voice mail if necessary. Your call will be returned within 24 business hours.

To schedule an appointment, we will request the following information from you:

  • your phone number
  • your diagnosis or a description of why you are seeking therapy
  • name of your referring physician; occasionally we need to contact the physician for clarification prior to your appointment
  • days or times of day that are convenient for you

We will verify that you have a current and valid prescription from your physician.

We will request insurance information:

  • If you have private insurance:
  • company name
  • member/policy number
  • group number
  • contact phone number for your insurance
  • your date of birth
  • policy holder’s name and date of birth, and employer, if different from patient
  • If you are an injured worker: we will ask you to provide a printed document showing your claim number at your first visit.
  • If you have Medicare: we will ask you to bring your Medicare card to your first appointment, as well as supplemental insurance card, if applicable.
  • If you have DSHS or another state funded plan, we may need your referral in advance of your appointment to start the authorization process or ask you to get a referral from your primary physician.
  • If you are uninsured or have a different situation, we will ask you to speak with our office manager to work out payment options.

We will call to verify your insurance coverage; however, we cannot quote your benefits to you, or explain the terms of your policy. We ask you to call your insurance company to ask for an explanation of your therapy benefits, deductible amounts, and coinsurance or co-payment.
